Here are a few ideas. Start with something you really want to know, but you think will come back pretty positive. This will give you and your coworkers courage to ask the tougher questions later. Remember keep it simple--one page, four of five questions, the Likert scale is fine, be sure to include a section for comments. That's it!

Before finalizing the survey, have a couple parents you trust complete the survey without help from you. Then listen to any confusion they may have had to help you revise the survey to make it as clear as possible. You'll be amazed at how much this "pilot" refines the survey. This step helps increase participation and gives you a far better chance of receiving accurate results.
